Sarasota Memorial is Moving to MyChart

Beginning Oct. 3, 2026, MyChart will be replacing SMH's FollowMyHealth portal. During the days leading up to this transition, we recommend that patients register for MyChart so your account is ready when the new portal launches.

Please note these important dates:

  • Sept. 21: Early access begins. Register for your new MyChart account (it will be view-only mode until the official launch Oct. 3).
  • Oct. 2: SMH's FollowMyHealth portal becomes read-only. No new health information will be added and messaging will be disabled. 
  • Oct. 3: MyChart officially launches with full functionality.

Want to save a copy of your FollowMyHealth information?

If you already use SMH's FollowMyHealth portal, your health information will transfer automatically to MyChart. You can download or save a personal copy of your data using the FollowMyHealth portal's export, print or send features.

  • From the FollowMyHealth mobile app: Go to the "My Health" tab, click "Send" or the export icon, and choose to print, email, or export your files.
  • From a computer: Log in to FollowMyHealth, go to the My Health tab, choose Summary, and click the Send option located at the top of the section. Select Print or Export to save the file as a PDF on your computer or choose Email to send a copy to your personal inbox.

FollowMyHealth is separate from the Sarasota Memorial First Physicians Group's MyHealthRecord patient portal. For more information regarding the MyHealthRecord transition to MyChart, please click here.

Patient Portal

MyChart: A New & Improved Patient Portal 

Sarasota Memorial is moving to MyChart, a new patient portal that will make it easier to access your health information and connect with your SMH care team.

You can sign up for MyChart now, but some features and health information will not be available until SMH’s MyChart patient portal officially launches on Oct. 3. 

Early access starts Sept. 21. During this time, you can:

  • Create a new MyChart account (even if you have used a different patient portal before). 
  • View future appointments scheduled on or after Oct. 3, 2026.
  • Receive notifications, such as appointment reminders for upcoming visits.
  • View past test results, visits and certain health records.
  • Set up proxy access (for a parent, caregiver or loved one) by sending an invite from your MyChart account or by contacting SMH for support.
  • During this time, MyChart will be in view-only mode. This means that messaging your care team, scheduling appointments, and other interactive features will not yet be available until Oct. 3, 2026.
  • You can sign up at mychart.smh.com/MyChart
  • The MyChart mobile app also is available for free download and use.

Full Access 

Starting Oct. 3, 2026, all MyChart features will be fully available.

With MyChart, you can:

  • View your medical records
  • Schedule and check-in for appointments online
  • Message your care team
  • Manage your medications
  • View radiology and test results
  • Get price estimates and manage bills
  • Manage proxy access
  • And much more!
