Patient Pre-registration

The pre-registration process helps us prepare for your upcoming hospital visit. While it is not required, pre-registration can expedite the check-in process. Please pre-register three business days prior to your scheduled service. If you have questions, call 941-917-7322.

 

Do I need a doctor to receive medical services?
Yes, a physician order is required to receive most medical services, with the exception of Emergency Room or Urgent Care services. If you need a physician referral, send email to healthline@smh.com or call 941-917- 7777 between 8 a.m. and 5 p.m.

How do I know if my insurance plan will cover my medical service?
Sarasota Memorial has contracts with most major medical insurance companies. See the list of Health Insurance Accepted, and verify your coverage with your insurance company. Co-payments are due at time of service.

Who will see the personal information I submit online?
Our Registration staff accesses your personal health information on a need-to-know basis to make your admission and stay as convenient as possible. Our Notice of Privacy Practices provides legal assurance and pertinent information regarding patient rights and confidentiality. You will also receive a copy of this brochure when you check in at Sarasota Memorial.

What should I bring on the day of my visit?
Please be prepared with your physician order, insurance cards, driver’s license or other photo identification. Co-payment is required at this time. Scheduled inpatients are advised to bring a copy of their Advanced Directives or Living Will for hospital records. Please leave any personal valuable items at home.
