Records Request & Release of Information

Sarasota Memorial maintains a medical record for every patient who visits the hospital, emergency room, clinic, and outpatient or laboratory setting. We have a legal and ethical responsibility to preserve the privacy of every patient’s protected health information. Before we release information, the patient or his/her legally qualified representative must give prior authorization.

To Request a Copy of Your Billing or Medical Records:

1. Download and complete the release form (see links below) and provide proof of identification with a valid, government-issued photo ID.

2. Submit the completed form and proof of identity in one of the following ways:

  • Send the documents via email toRequestMyMedicalRecord@smh.com

  • Drop off the documents to our Release of Information Office (first floor of the hospital, just beyond the C elevators); the office is open Monday - Friday, 8 a.m. to 5 p.m.

  • Mail the documents to this address:

  • Fax the documents to 941-917-2454.

Requests for medical records regarding continuation of care are sent directly to the physician at no cost. Physician offices may call 941-917-1108 to request medical records. All other requests are subject to MRO pricing. Fees are charged based on the type of record and the delivery method, either electronic or on paper.

Birth Certificates
We complete birth certificate information electronically for every infant born in the hospital, in compliance with Florida state requirements. Once our birth registrars enter the information, it is transmitted to the state automatically. Copies are available from the Sarasota County Health Department Vital Records Department at 2200 Ringling Blvd. in Sarasota, or at the following mailing address:

Mailed Subpoenas: Subpoenas served to the Hospital or one of its many entities must be in accordance with the Florida Rules of Civil Procedure 1.351(c) via certified U.S. Mail, FedEx, UPS or DHL with written confirmation of delivery or return receipt at the following address (so that they can be properly signed for):

Hand-served Subpoenas: Subpoenas hand delivered via Process Server for Medical Records, Depositions, Trials, and Standby of SMH personnel must be hand-delivered to the Hospitals agent at the new Legal Services Office at the following address:
