 Managing Heart Failure: Know What to Do and When to Act  

Join the Heart Failure Team at SMH as they discuss Heart Failure care at home, how to recognize flare-ups, and knowing when to act. This is a class for both patients and caregivers. The goal is to help patients stay stable, avoid hospital visits, and enjoy a better quality of life.
