1) After indicating that you are “Captain of the Ship”, the Electronic form changes in the following manner
A) The Home medications are no longer visible B) The ability to enter prescriptions is activated C) The check boxes before each Home and Active medication become active and available for clicking. D) Additional discharge medications may no longer be added to the bottom of the form
2) When checking medications from the Home and Active medication list, be very careful to:
A) Avoid checking the same medication if it appears in both the Home list and the Active list. B) Check the same medication if it appears on both lists in order to reinforce the instruction C) Check a substituted medication e.g. Zocor in the Active medication list even if you have checked Lipitor on the Home medication list.
3) A consultant should review the Discharge Medication Reconciliation form and then:
A) Go to the Order Entry Worksheet, change session type to “Discharge”, enter name of drug, fill in quantity and number of refills. The prescription will print after it is submitted. B) Leave it to the Attending to do all the prescriptions. C) Call the Attending and tell him what medications should be ordered.
4) If you would like to prescribe a medication that does not appear on the Home or Active list:
A) Tell the patient the medication is unavailable B) Write the prescription on your pad as you have in the past C) Scroll to the bottom of the form, click on one of the available boxes. This will activate several mandatory fields. Check the box after the medication to activate prescription writing. D) Log off of SCM and tell the nurse you want to do all this on paper!
5) If you would like to do the procedure from your office, you may enter prescriptions just as if you were working in the hospital.
