This service is a Medicare/Medicaid certified home
health agency that provides intermittent, skilled care in the home
as well as fee-for-service private duty care, 24 hours a day, seven
days a week.
*Our professional staff will develop a plan of care
with specific goals as directed by your physician.
As an integral part of the Sarasota Memorial Health Care System,
our goal is to provide customer oriented, efficient, comprehensive,
high quality health care in the home utilizing state-of-the-art equipment.
Skilled Nurse
- Provide skilled observation and assessment
- Instruct patient/family in disease management, diet, treatments and
medications
- Administer medications
- Provide treatments/procedures as ordered by the physician
Physical Therapist
- Provide restorative therapy, including therapeutic exercises, gait/transfer
training, prosthetic training and ultrasound
- Develop an individualized exercise plan
Occupational Therapist
- Provide ADL training to assist with independent living skills
- Provide restorative therapy for fine motor coordination
- Provide sensory treatments to assist with stroke recovery
- Construct orthotics and splints to improve function/decrease deformity
- Develop an individualized exercise program
Speech Therapist
- Treat voice, speech and language disorders
- Provide treatments to restore functional swallowing
- Develop an individualized exercise program
Medical Social Service Worker
- Counsel and assist with long-term health care planning
- Assist in the development of an in-home care system
- Explore alternatives to in-home care when necessary
- Serve as community resource liaison
Home Health Aides
- Provide bathing and personal care
- Assist with ambulating and transfers
- Assist with exercises, as directed by the therapist
- Private duty aides also assist with meals, shopping and transportation
Sarasota Memorial Home Care is licensed and certified by the State
of Florida to provide services to the residents of Sarasota County
and is accredited by the Community Health Accreditation Program
(CHAP). Community Health Accreditation

Services are provided directly by Sarasota
Memorial Home Care employees or individuals under contract with the
