da Vinci System Overview

The da Vinci Surgical System is the world's only robotic surgical platform designed to enable physicians to perform complex procedures through a series of 1-2 cm incisions. The da Vinci Surgical System has proven ideal for minimally invasive radical prostatectomy – the removal of the prostate to treat prostate cancer. Atrial Septal Defect

About four people in every 100,000 have Atrial Septal Defect (ASD), which is a heart condition present from birth. An ASD is a hole, or similar defect, in the wall of the atrium, the upper chamber of the heart where the wall between the right and left atria does not close completely during fetal development.

In fetal circulation an opening normally exists between the two atria (the upper chambers of the heart) to allow blood to bypass the lungs. This opening usually closes about the time a baby is born to allow it to begin to breathe and circulate oxygen-enriched blood. If the opening persists beyond birth, an ASD results. The ASD allows blood to continue to flow from the left to the right atria.

When no other congenital defect coexists, symptoms may be absent, particularly in children. It may be discovered during a routine physical, and confirmed with an echocardiogram. For most other patients, symptoms typically appear by age 30.

With a small-to-moderate atrial septal defect, a person may live a normal life span without symptoms. Larger defects may cause disability by middle age because of increased blood flow and shunting of blood back into the pulmonary circulation.

If left untreated, an ASD can result in heart failure or stroke. Individuals with ASD may also develop complications including infective endocarditis (a bacterial infection of the heart), heart failure and an abnormally fast heart rhythm known as atrial fibrillation.

When symptoms are reported, they usually involve fatigue and shortness of breath. Other symptoms may include: frequent respiratory infections, difficulty breathing (dyspnea), shortness of breath with activity and the sensation of feeling heart beat (palpitations). Because most people have no symptoms, most ASDs are discovered incidentally during medical examinations. There is no known way to prevent the defect, but some of the complications can be prevented with early detection.

Surgical Repair of an Atrial Septal Defect

ASD repair may not require treatment if there are few or no symptoms, or if the defect is small. Surgical closure of the defect is recommended if the ASD is large or if symptoms occur.

The traditional "open" approach to ASD repair requires that the surgeon cut through the breastbone -- the body’s natural protective structure for the heart -- and pull back the ribs to access the heart. This approach can prolong healing time, increase risk of infection, serious complications and even mortality.

Mitral Valve Disease

The mitral valve is a complex structure that controls blood flow through the left side of the heart. When open, the mitral valve allows blood to flow into the heart’s main pumping chamber, the left ventricle. When the left ventricle contracts to push blood through the body, the mitral valve closes to prevent blood from flowing back toward the lungs.

Sometimes the mitral valve is abnormal from birth or becomes damaged by infection. More often, mitral valve structures become damaged with age or from coronary artery disease.

Whatever the cause, an abnormal or damaged mitral valve cannot completely seal the heart’s left ventricle. With an incomplete seal, blood can escape the left ventricle and flow backwards through the mitral valve. This conflicting force can make the heart work harder, leading to further mitral valve damage and other potential complications such as congestive heart failure.

Surgical Mitral Valve Repair

Surgical repair involves delicate reconstruction of native valve tissues in order to restore proper function. The most common approach to repair requires the surgeon to saw open the breastbone and spread the ribs to gain direct access to the heart.

Cutting the sternum and opening the rib cage — the body’s natural protective structure for the heart — can prolong healing time, increase risk of infection, serious complications and even mortality.

Coronary Artery Disease

Heart disease refers to when arteries become clogged. Over time, fats, cholesterol and other substances build up in the walls of your arteries to form a type of plaque. Prostate Cancer

Prostate cancer is a disease in which malignant (cancer) cells form in the tissues of the prostate. The prostate is a gland in the male reproductive system located just below the bladder and in front of the rectum. It is about the size of a walnut and surrounds the urethra (the tube that empties urine from the bladder). The prostate gland produces fluid that makes up part of the semen.

Uterine Conditions

A wide variety of conditions affect the uterus, or womb. The uterus is a hollow, muscular organ which holds and feeds a fertilized egg. Traditionally, many conditions affecting the uterus are treated with hysterectomy – the surgical removal of the uterus. U.S. doctors perform about 600,000 hysterectomies every year, making it the second most common surgical procedure.

Hysterectomy may offer a life-saving treatment with serious conditions such as cancer or uncontrollable bleeding. In most cases, however, hysterectomy is an elective procedure performed to relieve chronic pain, bleeding or other disabling conditions. These may be caused by fibroids (noncancerous tumors); endometriosis (noncancerous growth of the uterine lining) or prolapse (falling or slipping of the uterus).

Traditional Hysterectomy

Hysterectomy can sometimes be performed through the vagina. However, when the uterus is enlarged – for example, from fibroids or cancer – an abdominal hysterectomy is preferable. This requires a 6-12 inch incision. In some cases, doctors also remove the ovaries because they are thought to release hormones contributing to the condition.
