Dear Potential Supplier:
In compliance with Sarasota Memorial Hospital’s current Vendor Policy we request that you read the enclosed literature and complete the enclosed registration forms.  Completion of these forms is required before being considered for our Approved Supplier List.  The enclosed forms should be submitted to the Supply Chain Management Department prior to arranging visits, meetings, demonstrations or product trials.  In addition, all new product introductions must be submitted to the Value Analysis Manager on a Vendor Information Sheet for review prior to being introduced to the hospital.

W-9 Request for Taxpayer Identification Number Certificate
