Sarasota Memorial’s ortho nurse navigator program guides patients and their families through the entire process of Total Joint Replacement, from the preparation for surgery through complete rehabilitation.
We strive to send patients home as soon as possible after their surgery. However, age and overall health - as well as physical barriers in the home, such as stairs - may make it best for a patient to go to a residential rehabilitation facility first.
Our case managers and ortho care navigator work with patients and families to ensure they have the equipment and support needed - everything from a walker to a hospital bed – to be able to care for themselves and move around safely when they leave our facility.
Whether patients go home or to another facility, we follow up to make sure they are well on the road to recovery.
