Release of Information

Sarasota Memorial maintains a medical record for every patient who visits the hospital,emergency room, clinic, or outpatient or laboratory setting. We have a legal and ethical responsibility to preserve the privacy of every patient’s protected health information.Before we release information, the patient or his or her legally qualified representative must give prior authorization.

To request a copy of your medical or billing records, please download and complete the release form and provide proof of identification with a valid, government-issued photo ID.Bring the form to our Release of Information Office on the first floor of the hospital, just beyond the C elevators, or mail or fax it to the address below. The office is open Monday through Friday, 8 a.m. to 5 p.m. We charge a copying fee of $1 per page, except for records needed for continuation of care, which we send directly to your physician at no cost. Physicians requesting records may call 941-917- 1108.

Birth Certificates
We complete birth certificate information electronically for every infant born in the hospital, in compliance with Florida state requirements. Once our registrar enters the information, it is transmitted to the state automatically. Copies are available from the Sarasota County Health Department Vital Records Department at 2200 Ringling Blvd. in Sarasota, or at the following mailing address:
