Release of Information:
Sarasota Memorial Hospital maintains a medical record for each patient seen in the hospital, emergency room, clinic, outpatient or laboratory setting.
Sarasota Memorial Health Care System has a legal and ethical responsibility to foster and preserve the privacy of patient protected health information. The release of information within the patient medical record (chart) requires prior authorization by the patient or their legally qualified representative before information is released, disclosed or made available for review.
To request a copy of your medical and/or billing records, you will be asked to complete a medical records release authorization form and provide proof of your identification with a valid government issued photo ID. You may download the Medical Record Request form by clicking on the link in the navigation on the left side of this page. The completed form may be mailed or faxed to our Release of Information Office, located in the hospital, first floor, just beyond the C elevators.
The office is open Monday – Friday, 8:00am – 5:00 pm and Saturday from 9:00am - 2:00pm.

Please note that there is a fee of $1.00 per page copied. Records needed for continuation of care will be sent directly to your physician at no cost.

Sarasota Memorial Hospital electronically completes birth certificate information for every infant born in the hospital. This is done in compliance with state of Florida requirements. Once the information has been entered by our registrar, it is automatically transmitted to the state. Copies are available from the Sarasota County Health Department:

Subpoenas for Deposition must be hand served to the address below. Subpoenas for Medical Records may be either mailed by a commercial delivery service (Certified U.S. Mail, FedEx, UPS, DHL, etc..) with written confirmation of delivery, or hand served to:
